Position Title: Faith Community Nurse

Status: Part Time
The Faith Community Nurse is a registered nurse with specialized knowledge who is called to ministry to provide whole person health promotion, disease preventions services, and spiritual needs for the Good News Rescue Mission guests and members. The major accountabilities and job activities of the Faith Community Nurse role are integrator of faith and health, health educator, personal health counselor, referral agent, developer of support groups.
1. Personal Health Counselor
a. Provides individual health counseling related to health maintenance, disease prevention or illness patterns.
b. Documents client assessment, nursing diagnosis, interventions and outcomes while maintaining confidential client record in accordance with the policy on documentation.
c. Makes visits as needed providing health counseling, education and spiritual presence/support.
d. Promotes stewardship of the body emphasizing self care of the whole person
e. Collaborates with Guest Services Manager and Director of Operations to plan for health education programming
f. Communicates with other health professionals as needed to meet the health needs of clients
2. Health Educator
a. Utilizes information from needs assessments of the guests and members of GNRM and surrounding community in planning for education programs.
b. Prepares, develops and/or coordinates educational programs based on identified needs for healthier lifestyles, early illness detection and health resources.
c. Maintains records of educational programs, including objectives, content, evaluation, attendance and budget.
d. Documents individual educational assessment diagnosis, interventions and outcomes.
e. Provides a brief summary evaluation of educational programs to the Guest Services Manager and Director of Operations with a cc to the Controller and Executive Director.
f. Networks with appropriate resources in the community to secure educational program resources.
g. Provides consultation and acts as a health resource to other staff of the Mission.
h. Developer of Support Groups
i. Develops and/or facilitates support groups based on identified needs and resources.
j. Identifies available support groups in the community that could resource the congregation.
k. Refers and documents client participation in designated support groups.
3. Referral Agent
a. Provides and documents referrals to health care services and resources within the GNRM and external community
b. Collaborates with community leaders and agencies to facilitate effective working relationships while identifying new health resources.
c. Develops community contacts, in cooperation with the Administration, Volunteer, Fund Raising and Operations department Directors, in order to secure resources and services to meet the needs of members of the Mission. All contracts from such contacts will be kept in the Controller’s office of Administration and approved by the Controller prior to entering into the agreement.
d. Networks with other faith community nurses and professionals.
e. Manages department volunteers which includes: supervising and assignment of volunteer work and setting the volunteer schedule. Works closely with the Volunteer Coordinator regarding Mission volunteer policies and procedures.
4. Health Advocate
a. Encourages clients to avail themselves of services, which will enhance their overall wellbeing, assisting the clients in identifying values, and choices, which encourage them to be more responsible for their health status. Perform assessment, blood pressure and vital signs as needed.
b. Assists client and client families in making decisions regarding their health, medical services, treatments and care facilities as well as documenting assessments, diagnosis, interventions and outcomes.
c. Identifies, communicates, and works cooperatively with community leaders and agencies to meet health needs of members of GNRM and surrounding community. Evaluation of client needs and ability of the Mission to care for a patient being discharged from local hospitals to GNRM.
Minimum Job Requirement:
Must have a committed personal relationship with Jesus Christ and belong to a Christian body of believers.
Demonstrates appropriate behaviors and character traits of a Christian.
A demonstrated passion for ministry, advocacy and issues surrounding homelessness and poverty.
Agrees with and can sign the Good News Rescue Missions Statement of Faith, Statement on Marriage and be able to share that faith and knowledge with staff and clients.
Current license as a registered nurse in CA, BS Preferred, five years clinical nursing experience.

WORKING CONDITIONS AND PHYSICAL EFFORT:
The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Pleasant working environment; maybe exposed to cold occasionally. The noise level in the work environment is usually moderate. Employee may come into contact with irate and irrational clients who may or may not be under the influence of mind-altering substances such as cocaine, marijuana, alcohol, etc., or be mentally or physically disabled. Employee may be regularly exposed to some risk of communicable disease from guests of facility, exposure to TB and other respiratory diseases.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to walk, talk and hear. The employee is occasionally required to stand and regularly required to sit, manual dexterity necessary in using hands to finger, handle, feel, type on computer keyboard. The employee required to reach with hands and arms. The employee must occasionally lift and/or move up to 25 pounds up stairs. Specific vision abilities required by this job include distance vision, color vision, depth perception and peripheral vision. The employee is required to have close visual acuity to perform an activity such as viewing a computer terminal.
